At Three Rivers Medical Associates, we’re proud to introduce Heather Green, a dedicated Nurse Practitioner whose career has been shaped by a deep commitment to helping patients navigate complex health challenges with confidence and clarity. With more than 15 years of experience as a nephrology nurse practitioner, Heather brings a strong foundation in chronic disease management and patient-centered care to her role as an internal medicine provider.
Heather earned both her Bachelor of Science in Nursing (2005)and Master of Science in Nursing (2010)from the University of South Carolina. Early in her career, she found herself working in nephrology, a field that quickly proved to be both intellectually engaging and deeply rewarding. Kidney health touches nearly every system in the body, influencing blood pressure, electrolyte balance, heart health, and fluid regulation. The complexity of these interconnected systems offered Heather endless opportunities to learn, investigate, and support patients facing serious medical conditions.
Over time, Heather developed a deep appreciation not only for the science behind kidney care, but also for the relationships that naturally develop when managing chronic illness. Many patients with kidney disease require long-term guidance and support as they navigate complicated treatment decisions, lifestyle adjustments, and evolving health needs. Building those relationships and helping patients feel supported through each stage of their care became one of the most meaningful aspects of her work.
Heather spent 2011 through 2025 working as a Nephrology Nurse Practitioner, becoming one of only eight Certified Nephrology Nurse Practitioners in the state of South Carolina. During those years, she gained extensive experience managing complex conditions including
chronic kidney disease, hypertension, and other multisystem medical issues that require thoughtful, coordinated care.
Today, Heather brings that depth of experience into internal medicine, where her approach focuses on collaboration, education, and practical solutions that patients can realistically incorporate into their daily lives. Medicine can often feel overwhelming for patients, especially when they are managing multiple diagnoses, medications, or lifestyle changes. Heather works to simplify that complexity by helping patients clearly understand what is happening with their health and what steps they can take moving forward.
Her goal is to ensure that patients leave each visit feeling informed, supported, and confident in the plan they have created together. Rather than feeling like medical decisions are happening around them, Heather strives to make patients active participants in their care, building trust and partnership along the way.
One of the most rewarding aspects of Heather’s career has been helping patients regain a sense of control during difficult moments in their health journey. She recalls a patient with advancing kidney disease who initially felt overwhelmed by the prospect of dialysis and feared that it meant the end of the life he knew. Through ongoing conversations, education, and small, manageable changes, Heather helped him better understand his options and focus on what
mattered most to him. Over time, his outlook shifted from fear to confidence, and he no longer felt like he was simply waiting for something bad to happen. Experiences like this continue to reinforce why Heather chose this profession.
Outside of medicine, Heather enjoys spending time at home in Fairfield County with her husband and their two boys. She enjoys gardening, cooking, and reading; hobbies that reflect her appreciation for balance, well-being, and time spent with family.
